Register at Sammamish Diabetes and Lipid Clinic, PLLC

Notwithstanding any previous relationship with Dr. Stewart at either Pine Lake Family Medicine or at Swedish Physicians Pine Lake, all patients wishing to be seen at Sammamish Diabetes and Lipid Clinic, PLLC, will need to register as patients at the clinic, since this is a new clinic, with a different business license, different Tax ID Number, and different insurance contracts.

Financial Policies

In keeping with our policy of eliminating unnecessary overhead expense, we will not be sending bills to patients for allowed charges that insurance did not pay due to co-pay or deductibles.  Instead, we ask that every patient authorize us to directly bill his or her credit card or debit card for co-pay at the time of service and for co-insurance and deductible amounts when we receive the explanation of benefits from their insurance companies.  Patients will be notified of the amount billed to their credit or debit cards via email at the time it is charged.   Patients who do not have credit or debit cards may leave a signed, blank check to cover this obligation.

Yearly Subscription Fee

Secure internet messaging, 24/7 web access to your chart, and virtual visits (when appropriate) are all services that are not covered by insurance companies at this time.  To cover the expense of implementing these additional services, all patients are expected to pay a yearly subscription fee of $50.00 for one patient or $75.00 for  the patient and all family members living in the same household.  These fees will be due as soon as these services are implemented, which is expected to be by mid-September, 2007. They will be renewed yearly.  Patients who are not able to use these services or who choose not to use them should seriously consider whether this is the right practice for them.   Patients who leave the practice are entitled to a pro-rated refund of the yearly fee as of the time I am notified of their departure, provided that all other financial obligations to the practice have been met.
