Make an Appointment
We offer on-line appointment services via Appointment Quest. The first time you use this service, you will have to register with them.
Please look at the time allowed for the the different types of appointments and schedule your visit based on the time you expect to need for your visit. When you schedule your appointment, you will have an option to enter the real purpose of your visit.
If you are unable to find an available time that meets your needs, please either send Dr. Stewart an Email describing the problem DonS@SammamishDiabetesAndLipid.org or call the office number (425) 996-7047, and leave a message describing your needs. For urgent issues, you can reach Dr. Stewart on his cell phone (206) 450-1222, but please understand that you may be interrupting him with a patient.
As part of ongoing quality research projects, we ask every patient at Sammamish Diabetes and Lipid Clinic to complete the online HowsYourHealth? questionnaire every 6 months. The questionnaire takes about 10 minutes. If you have not completed the questionnaire in the last 6 months, please do it now.
